Job summary

Aventura at Assumption Village in North Lima, OH is seeking a Certified Occupational Therapy Assistant (COTA) to join our in‑house therapy team. We offer full‑time benefits and PRN opportunities with a supportive interdisciplinary staff.

You’ll work under the supervision of an Occupational Therapist to help residents improve independence and daily living skills. Flexibility in scheduling, competitive compensation, and a collaborative work environment are available for eligible staff.

Qualifications

• Active Ohio COTA license is required and must be in good standing.
• Experience in geriatric or LTC/SNF settings is preferred but not required.
• Strong communication, organizational and teamwork skills are essential.
• Familiarity with Net Health or similar therapy management systems is a plus.
• Ability to work effectively as part of an interdisciplinary team.

Responsibilities

• Provide skilled occupational therapy assistant services under OT supervision.
• Assist residents with activities of daily living and functional activities.
• Monitor progress and report findings to the supervising OT.
• Complete timely and accurate documentation of care provided.
• Educate residents and families on therapy goals and techniques.
• Collaborate with nursing and other care team members to support resident independence.

What You\'ll Do

As a COTA, you\'ll work under the supervision of a licensed Occupational Therapist to provide skilled therapy services that help residents improve their independence, daily living skills, mobility, function, and overall quality of life.

Responsibilities include:

• Providing individualized therapeutic interventions

• Implementing treatment plans under the direction of the supervising OT

• Assisting residents with activities of daily living and functional activities

• Monitoring and reporting resident progress and response to treatment

• Educating residents, families, and caregivers on therapy goals and techniques

• Completing timely and accurate documentation

• Communicating resident progress and concerns to the supervising OT

• Collaborating with nursing and the interdisciplinary care team

• Providing compassionate, resident-centered care

What We\'re Looking For

• Active Ohio COTA license

• License in good standing with applicable licensing boards and regulatory agencies

• LTC/SNF or geriatric experience preferred, but not required

• Net Health experience is a plus

• Strong communication, organizational, and teamwork skills

• Ability to work effectively as part of an interdisciplinary team

• Passion for helping residents reach their highest level of independence
