Job summary

Blue Stone Therapy in New Hampton, IA, is seeking an Occupational Therapy Assistant to provide rehabilitation services to adult and geriatric patients at Accura Healthcare of New Hampton, IA, with travel to Faith Lutheran Home in Osage as needed.

The COTA will implement treatment programs, document progress, and collaborate with an interdisciplinary team to support positive patient outcomes in SNF and rehab settings.

Qualifications

• Graduate of an accredited OTA program.
• Current OTA license or eligible in the applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehab preferred.

Blue Stone Therapy is seeking an Occupational Therapy Assistant to provide rehabilitation services to adult and geriatric patients at Accura Healthcare of New Hampton, IA. This position will support Faith Lutheran Home in Osage, IA with travel between locations as needed.

Responsibilities

• Provide skilled occupational therapy treatment in accordance with established plans of care.
• Implement individualized treatment programs focused on functional and measurable outcomes.
• Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings.
• Monitor patient response to treatment and communicate progress with the supervising Occupational Therapist.
• Maintain timely, accurate documentation to support quality care and reimbursement standards.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.

Requirements

• Graduate of an accredited Occupational Therapy Assistant program.
• Current Occupational Therapy Assistant license (or eligibility for licensure) in applicable state.
• Experience in skilled nursing, long‑term care, or post‑acute rehabilitation preferred.
• Strong communication, organization, and clinical reasoning skills.
