Job summary

Blue Stone Therapy is seeking an Occupational Therapy Assistant (COTA) to provide rehabilitation services to adult and geriatric patients at Eventide on Eighth in Moorhead, MN, with occasional support at a Fargo facility. The role emphasizes collaborating with an interdisciplinary team to achieve meaningful patient outcomes.

Qualified candidates will have an accredited OTA program credential, active license or eligibility, and strong communication and organizational skills to support quality

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current OTA license or eligibility for licensure in the applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong communication, organization, and clinical reasoning skills.
• Commitment to patient-centered care and professional standards.

Responsibilities

• Provide skilled occupational therapy treatment in accordance with established plans of care.
• Implement individualized treatment programs focused on functional and measurable outcomes.
• Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings.
• Monitor patient response to treatment and communicate progress with the supervising Occupational Therapist.
• Maintain timely, accurate documentation to support quality care and reimbursement standards.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.

Occupational Therapy Assistant (COTA) - Moorhead, MN

Blue Stone Therapy is seeking an Occupational Therapy Assistant (COTA) to provide rehabilitation services to adult and geriatric patients at Eventide on Eighth in Moorhead, MN. (This position will also support a facility in Fargo, at Bethany on University one day per week)

What You’ll Do

• Provide skilled occupational therapy treatment in accordance with established plans of care
