Job summary

Northshore Home Health Care is seeking an Occupational Therapist to evaluate and provide rehabilitation services to patients requiring OT. You will develop individualized care plans and deliver skilled interventions per physician orders, while monitoring progress and collaborating with a multidisciplinary team.
