Job summary

FOX Rehabilitation is growing in Thibodaux, LA and seeks licensed Occupational Therapists to deliver one-on-one care in homes or Senior Living Communities. You’ll work within a Geriatric House Calls™ model, building strong patient relationships while maintaining your own schedule and flexibility.

Telehealth and other virtual care options may be incorporated, offering greater access and continuity of care. OT clinicians will evaluate, plan, and treat to improve mobility and independence in older

Qualifications

• Valid OT license in the state of practice or eligibility to obtain
• Degree from an accredited OT program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one occupational therapy to older adults where they live (private residences or senior living communities)
• Perform evaluations and develop individualized plans of care
• Deliver therapy sessions to support wellness, mobility, independence
• Document effectively within FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access

Physical Requirements

FOX clinicians must be able to meet the physical demands of in-home and community-based care, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly

• Performing CPR

• Providing moderate to maximumassistancewith patient transfers and bed mobility

• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities

• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model

• Wearing PPE whenrequired
