Role Overview: As an OT in this role, you will be an integral part of our innovative Take Your Therapist Home Program.

What is Take Your Therapist Home?

In a world of shorter SNF stays, TYTH is a unique program where clinicians work directly with patients in SNF and home health settings, providing continuity of care and a greater impact on patient outcomes.

You will primarily work in home health with dedicated time and caseload support for SNF. Your coverage area will be localized around the SNF, with a standard workweek schedule.

Rivetus has served the geriatric Post-Acute community since 1998, providing continuity of care across Skilled Nursing, Home Health, and Outpatient settings.

Schedule: Monday - Friday, Full Time

Minimum Qualifications:

• Successful completion of an accredited Occupational Therapy program
• Licensure as required by Michigan state law

• Professional judgment and initiative in problem-solving
• Flexibility and initiative in the work environment
• Effective reaction to patient and family needs
• Physical ability to lift and transfer patients weighing 80 to over 300 pounds, using recommended devices
