Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our growing team in a SNF setting. As an Occupational Therapist, you will evaluate and treat patients following an individualized plan of care, and work collaboratively with facility staff to ensure high-quality outcomes.

You will document evaluations and progress notes, develop measurable goals, re-evaluate progress, and provide supervision to Occupational Therapy Assistants as needed.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
