Job summary

Cortlandt Healthcare in Cortlandt Manor, NY is a 120-bed sub-acute facility hiring a full-time Occupational Therapist to join our rehabilitation team. On-site interviews with our Director of Rehab and facility leadership ensure you meet the team you will work with.

Qualifications

• Degree in Occupational Therapy from an accredited program.

Responsibilities

• Evaluate referred residents to determine medical status, functional ability, limitations, precautions, and rehabilitation potential.
• Build individualized treatment plans with measurable, achievable goals based on each resident’s capacity and tolerance, under physician direction.
• Deliver skilled occupational therapy interventions focused on ADL performance, safety, and return to prior level of function.
• Complete evaluations, weekly progress notes, recertifications, and discharge documentation accurately and on time.
• Educate residents, families, and caregivers on therapy techniques, safety, and carryover strategies.
• Collaborate with PT, SLP, nursing, dietitians, and social work to deliver patient centered care and support discharge readiness.
• Recommend adaptive equipment, durable medical equipment, and environmental modifications.
• Supervise Occupational Therapy Assistants and students in accordance with the state practice act.
