Sigma Health Rehab is seeking a Full-time Occupational Therapist (OT) to work in a Skilled Nursing Facility in New York. A $6,500 sign-on bonus is offered to eligible candidates. Applicants must graduate from an accredited OT program and hold a New York State OT license, with preference for SNF/geriatric experience.

Qualifications

• Graduate of an accredited Occupational Therapy Program
• Current New York State OT License
• Experience in a skilled nursing or geriatric care setting preferred, new grads encouraged

Responsibilities

• Evaluate residents and develop OT treatment plans meeting individual needs
• Select activities enabling residents to perform daily tasks independently
• Observe, assess, and document OT progress with detailed reports
• Maintain resident confidentiality in information processing
• Maintain cooperative relationships within the healthcare team
