Occupational Therapist, OT - Per Diem

BAYADA Home Health Care is seeking an experienced Occupational Therapist, OT for a per diem home health visit position at our Lehigh Senior Living Office in Allentown, PA.

This office services adult and geriatric patients in Senior Living Facilities in East Stroudsburg, PA. As a home care Occupational Therapist, you will be a key member of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Perform comprehensive evaluations of patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
3. Assess cognitive, psychological, and motor performance; develop individualized intervention plans.
4. Evaluate home environments for hazards or barriers to independence.
5. Identify equipment needs and environmental modifications to improve outcomes.
6. Continuously assess and update care plans; collaborate with nursing staff in multidisciplinary planning.
7. Educate patients and families on rehabilitative activities to promote health, safety, and independence.
8. Document observations, interventions, and evaluations accurately using electronic devices.

Qualifications:

1. Current PA Occupational Therapist license.
2. At least one year of recent OT experience; home care experience preferred but not required.
3. Graduation from an accredited occupational therapy program.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Proficiency with computers; EMR experience preferred.
