Occupational Therapist - Senior Living Home Health Visits ($2,000 Sign-on Bonus)

Join BAYADA Home Health Care as an Occupational Therapist providing home health visits in the Newark/Glasglow territory. This role offers a $2,000 Sign-on Bonus for full-time positions.

Position Overview

As a home care Occupational Therapist, you will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing to clients, enabling them to remain at home.

Responsibilities

1. Make home visits to clients within designated geographic areas.
2. Conduct comprehensive evaluations of patients' functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
3. Assess cognitive, psychological, and motor performance; develop individualized intervention plans.
4. Evaluate home environments for hazards or barriers to independence.
5. Identify equipment needs and environmental modifications to improve outcomes.
6. Continuously assess and update care plans; collaborate with nursing staff.
7. Educate patients and families on rehabilitative activities to promote health and safety.
8. Document observations, interventions, and evaluations accurately using electronic medical records.

Qualifications

1. Current Delaware Occupational Therapist license.
2. At least one year of recent OT experience; home care experience preferred but not required.
3. Graduation from an accredited occupational therapy program.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Proficiency with computer systems; EMR experience preferred.
