Job summary

Liberty Health Weldon, NC, is seeking an Occupational Therapist (OT) to join the Liberty Commons of Halifax County team. You will conduct rehab services, develop individualized treatment plans, and work within a multidisciplinary group to enhance patient outcomes in a SNF setting.

The ideal candidate is licensed in NC with a graduate therapy degree, and has prior SNF rehab experience. In-house therapy with local management support, and a sign-on bonus of $10,000 are offered.

Qualifications

• Graduate of an accredited therapy program.
• Must be licensed in the state of North Carolina.
• SNF rehab experience preferred.

Responsibilities

• Develop individualized treatment plans for patients.
• Collaborate with a multidisciplinary team to coordinate care.
• Provide in-house occupational therapy within the SNF setting.
