Job summary

Sweetwater Care is seeking a licensed Occupational Therapist to perform treatments and exercises to aid patient recovery. You will tailor personal treatment plans, educate patients on exercises, and supervise support staff while maintaining thorough records and discharge summaries.

Responsibilities include collaboration with the Rehabilitation Services Director, use of evidence-based practices, and adherence to regulatory requirements. Active state licensure is required.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms.
• Diagnose movement dysfunction by observing patients as they move.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records and track goals.
• Use standardized assessments and evidence-based practice to support clinical interventions.
• Assess progress and adjust treatments per regulatory and clinical requirements.
• Evaluate treatment outcomes and modify services accordingly.
• Provide consultation and counseling to patients, families, and caregivers.
• Communicate progress and plans with supervisor and health team.
• Provide discharge summaries per regulatory requirements.
• Document all required information in the electronic system and maintain patient records.
• Participate in trainings, in-services and conferences.
• Supervise and support PT assistants, rehab aides and students.
• Adhere to company policies and code of conduct.
• Maintain active State Licensure.
