Qualifications

• California license as an Occupational Therapist is required
• Minimum 2 years of healthcare experience, preferably in a skilled nursing facility
• Proficient with electronic medical records

Responsibilities

• Evaluate residents' physical, cognitive, and psychosocial abilities and functional limitations using standardized assessments
• Develop and implement individualized treatment plans based on assessments and goals
• Provide therapeutic interventions including ADL training and cognitive retraining
• Monitor progress and adjust plans as needed
• Collaborate with physicians, nurses, PTs, and SLPs to coordinate care
• Educate residents and families on goals and home exercises
• Maintain accurate documentation of OT services
• Participate in team meetings and quality improvement initiatives
• Stay current with OT practice standards and regulations
