Job summary

You will develop individualized care plans, deliver therapeutic interventions to improve functional independence, educate patients and families on adaptive techniques and equipment, and participate in discharge planning while documenting in the EMR.

Responsibilities

• Provide evidence-based evaluation and treatment to patients across acute, inpatient rehab, and/or outpatient settings.
• Develop individualized care plans and deliver therapeutic interventions to improve functional independence.
• Educate patients and families on adaptive techniques and equipment.
• Collaborate with physicians, nurses, PT/SLP, and social work; contribute to discharge planning.
• Document in the EMR and support quality, safety, and performance improvement initiatives across the MedStar system.
