Job summary

MENTORHILLS is seeking an Occupational Therapist to evaluate and treat patients, communicate with families and physicians, and maintain clinical documentation. The role includes supervision of OT Assistants and training for facility staff, with responsibilities spanning evaluation, treatment, and discharge planning.

Education requires a Bachelor’s in Occupational Therapy with state licensure; Proficiency in Casamba and Point Click Care is preferred.

Qualifications

• Bachelor's degree in Occupational Therapy; advanced degree preferred.
• Licensed to practice OT; meet state CE requirements.
• Proficient with Casamba and Point Click Care software.
• Ability to read and interpret policies, procedures, and clinical data.
• Strong communication with families, physicians, and health teams.

Responsibilities

• Evaluate patients within 24 hours of referral and develop treatment plans.
• Treat patients per physician-ordered plan and document progress.
• Supervise OT Assistants and train restorative staff as needed.
• Record evaluations, treatment notes, and discharge planning information.
• Communicate patient progress to supervisor and care team.
