Job summary

Diamond Care Group seeks a skilled Home Health Physical Therapist to deliver personalized in-home PT services, helping patients regain mobility and independence after illness or surgery. You will perform evaluations, create treatment plans, and guide patients through exercises in a home setting.

The role requires current FL PT license, accredited PT education, and CPR certification, with 1–2 years in Home Health preferred. A valid driver’s license and knowledge of OASIS are essential.

Qualifications

• Current Physical Therapist license (FL)
• Graduate of accredited PT program
• 1–2 years of Home Health experience preferred
• Strong communication skills; ability to work independently
• Possesses and maintains CPR certification
• Valid driver’s license and reliable transportation
• Knowledge of OASIS
• Experience with EMR systems such as Kinnser

Responsibilities
