Job summary

Apple Valley Health & Rehab in Yakima, WA is seeking a compassionate Occupational Therapist (OT) to join our team on a per diem basis. The OT will provide skilled therapy to residents, helping them achieve independence and improved function.

The ideal candidate is collaborative, patient-focused and experienced in rehabilitation and skilled nursing settings, with Washington licensure or eligibility and CPR/BLS certification. Per diem flexibility is available and compensation is DOE.

Qualifications

• Current OT license in Washington or eligibility
• Graduate of an accredited OT program
• CPR/BLS certification, or ability to obtain as required
• Experience in skilled nursing or rehabilitation settings preferred
• Strong communication and documentation skills
• Ability to work independently within a collaborative team

Responsibilities

• Evaluate residents to identify functional limitations and goals
• Develop and implement OT treatment plans
• Provide skilled therapies to improve ADLs, mobility, strength and coordination
• Monitor progress and adjust plans as needed
• Document evaluations, treatments and discharge information
• Collaborate with nursing, PT, SLP and physicians
• Educate residents and families on therapy techniques and home programs
• Maintain compliance with regulations and facility policies
• Promote safety, independence and quality of life

Apple Valley Health & Rehab is seeking a compassionate and motivated Occupational Therapist (OT) to join our team on a per diem basis. The Occupational Therapist will provide skilled therapy services to residents, helping them achieve greater independence, improve functional abilities, and maintain the highest possible quality of life.

The ideal candidate is a collaborative, patient-focused professional who enjoys working in a rehabilitation and skilled nursing environment.

Responsibilities:

• Evaluate residents to identify functional limitations, occupational performance needs, and individualized treatment goals.

• Develop and implement individualized occupational therapy treatment plans.

• Provide skilled therapeutic interventions to improve strength, coordination, mobility, activities of daily living (ADLs), and overall functional independence.

• Monitor resident progress and modify treatment plans as clinically appropriate.

• Document evaluations, treatment sessions, progress, and discharge information accurately and in a timely manner.

• Collaborate with nursing staff, physical and speech therapy, physicians, caregivers, and other members of the interdisciplinary team.

• Educate residents, families, and caregivers on therapeutic techniques, adaptive strategies, and appropriate home or facility programs.

• Maintain compliance with applicable federal and state regulations, professional standards, and facility policies.

• Promote resident safety, dignity, independence, and quality of life.

Qualifications:

• Current Occupational Therapist (OT) license in the State of Washington, or eligibility for Washington licensure.

• Graduate of an accredited Occupational Therapy program.

• Current CPR/BLS certification, or ability to obtain as required.

• Experience in skilled nursing, rehabilitation, long-term care, or a related setting is preferred.

• Strong clinical, communication, organizational, and documentation skills.

• Ability to work independently while collaborating effectively with an interdisciplinary team.
