Five Points Nursing and Rehabilitation of Pflugerville is seeking a compassionate Occupational Therapist (OT) to provide PRN clinical care focused on improving residents’ independence and safety in daily activities. You will assess, plan, and implement therapies to maximize ADLs and IADLs while collaborating with the rehab team.
The role requires current Texas OT licensure, experience in SNF or long-term care, and the ability to educate patients and families.
We are seeking a compassionate and flexible Occupational Therapist (OT) to join our in-house team of outstanding professionals on a PRN basis. In this role, you will provide person-centered care focused on improving residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes.
Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life
Assess and interpret patient evaluations and test results
Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
Assess patients’ home and living environments and recommend modifications based on their needs and capabilities
Recommend adaptive equipment and provide training to support independence and safety
Ensure all treatments are delivered in accordance with the established plan of care
Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team
Provide clinical education and support to patients, family members, and caregivers
Collaborate with interdisciplinary team members to ensure high-quality patient care
Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
Comply with all applicable rules, regulations, and procedures
Attend required meetings as designated by the Director of Rehab
Active and valid Texas Occupational Therapy license
In good standing with all regulatory agencies and licensing boards
Skilled Nursing Facility (SNF) or long-term care experience preferred
Current knowledge of occupational therapy treatment practices
Experience with Net Health Electronic Documentation System preferred
Ability to manage patients with varying personalities and needs
Strong communication, collaboration, and clinical decision-making skills
