Qualifications

Responsibilities

• Evaluate patients and develop personalized care plans focused on functional independence, safety, and quality of life
• Deliver care through activities of daily living training and adaptive techniques
• Assist patients in improving daily tasks such as dressing, bathing, grooming, and meal preparation
• Collaborate with interdisciplinary teams, caregivers, and nursing staff to support outcomes
• Track progress, document clinically, and adapt treatment plans as needed

• Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals
