Anew Home Health is hiring Occupational Therapists to join our Indiana-based home health team. You will deliver skilled OT services to patients in their homes under minimal supervision, with a focus on maximizing independence and quality of life.

We offer competitive per-visit rates, mileage reimbursement at 0.58 per mile, and a Monday–Friday schedule in Lafayette area, with PRN hours available to fit your lifestyle.

Qualifications

• Must be a licensed Occupational Therapist.
• Home health experience preferred.
• Excellent patient care and communication.

Responsibilities

• Provide skilled OT services in patients' homes.
• Evaluate patients and develop initial and ongoing treatment plans.
• Develop discharge plans and supervise assistants as needed.
• Contribute to case management and ensure quality patient care.

Per-Visit Rates

Rates paid per visit as follows:

• Start of Care (SOC): $150.00
• Resumption of Care (ROC): $115.00
• Recertification: $95.00
• Reassessment: $70.00
• Routine Visit: $70.00
• SCIC Visit: $95.00
• Discharge Visit: $80.00
• Discharge Visit – Offsite: $40.00
• Evaluation: $85.00

Mileage Reimbursement

• $0.58 per mile

Non-Visit Productivity (i.e. education, staff meetings)

• $45.00 per hour

The Occupational Therapist implements, with minimal supervision, skilled occupational therapy services in accordance with the principles and practices of occupational therapy and within Anew Home Health policies and procedures. Evaluates patients, develops initial and ongoing treatment plans, develops discharge plans, supervises assistants, contributes to case management, and provides quality patient care.
