Job summary

Creative Solutions in Healthcare, Inc. is seeking a compassionate Occupational Therapist (OT) to join our in-house team on a PRN basis. You will evaluate residents, develop plans, and collaborate with an interdisciplinary team to enhance independence and safety in daily activities.

Ideal candidates hold a Texas OT license, have SNF/long-term care exposure, and demonstrate strong communication and patient-centred care. This role offers flexible scheduling and in-house rehab opportunities.

Qualifications

• Active and valid Texas Occupational Therapy license.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on ADLs & IADLs.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADL/IADL performance.
• Assess home environments and recommend modifications.
• Recommend adaptive equipment and provide training for independence and safety.
• Deliver treatments per the plan of care and monitor responses.
• Collaborate with the interdisciplinary team on patient care.
• Provide clinical education to patients, family and caregivers.
• Complete documentation timely and in compliance with policies.
• Attend required Rehab Director meetings.

Join Our Team as a (PRN) Occupational Therapist (OT)

We are seeking a compassionate and flexible Occupational Therapist (OT) to join our in-house team of outstanding professionals on a PRN basis. In this role, you will provide person-centered care focused on improving residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes.

Your Impact as an Occupational Therapist (OT)

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life
• Assess and interpret patient evaluations and test results
• Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Assess patients’ home and living environments and recommend modifications based on their needs and capabilities
• Recommend adaptive equipment and provide training to support independence and safety
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical education and support to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab
