Job summary

JourneyCare Home Health is seeking an Occupational Therapist for a PRN, field-based position in Dolton, IL and surrounding areas. The OT will manage a patient caseload, provide skilled therapy, and coordinate care in the home setting.

Responsibilities include developing treatment plans, communicating with physicians, and teaching/ retraining patients in daily activities. Strong preference for candidates with prior clinical OT experience and valid CPR certification.

Qualifications

• Graduate OT curriculum accredited jointly by the AMA/COA and AOTA.
• Registered with the NBCOT examination.
• At least two years of clinical OT experience (preferred).
• Must have reliable transportation, current driver's license and insurance.
• Maintains current CPR certification.

Responsibilities

• Provides occupational therapy services to patients per physician's plan of care.
• Develops treatment programs and sets goals for improved function.
• Teaches new skills or retrains patients in activities affected by illness or injury.
• Fabricates splints and instructs on adaptive equipment use to improve function.
• Trains patients in use of prosthetic and/or orthotic devices.
• Maintains clinical records and reports changes to the physician as required.
• Maintains communication with the care team for coordinated care.

JourneyCare Home Health is currently seeking a dedicated Occupational Therapist to join our home health team on a PRN basis. This field-based position is responsible for managing a caseload of patients in Dolton, IL and surrounding areas, providing skilled services, coordinating care, and supporting patients on their road to recovery in the comfort of their homes.

Location: Dolton, IL and surrounding areas.

Hourly: $42.79 - $55.29

Schedule: PRN

What You’ll Do:

• Provides occupation therapy services to patients according to a written physician's plan of care and as defined in the state Occupational Therapy Practice Act.
• Develops treatment program and establishes goals for improved function. Communicates plan of care to referring physician and other Organization professionals as needed.
• May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury, in accordance with our policy.
• May fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function.
• May train patient in the use of prosthetic and/or orthotic devices.
• Maintains appropriate clinical records, clinical notes, and reports to the physician any changes in the patient’s condition. Submits these records in accordance with our policy.
• Maintains contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care.

Qualifications:

• Graduate of an occupational therapy curriculum accredit�ed jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
• Registered with the National Registration Examination of the American Occupational Therapy Association.
• At least Two (2) years of clinical experience as an occupational therapist. (Preferred)
• Must have reliable transportation, current driver's license and appropriate automobile insurance
• Complies and maintains current CPR certification.
