Job summary

Devine Health and Rehabilitation in Texas seeks a compassionate Occupational Therapist (OT) for a PRN role on our in-house rehab team. You will help residents gain independence and safety through meaningful daily activities and functional goals.

You will evaluate patients, plan individualized OT interventions for ADLs/IADLs, and collaborate with the care team to ensure high-quality outcomes. Texas OT license required; SNF/LTC experience and Net Health experience preferred.

Qualifications

• Licensed OT in Texas and in good standing with regulatory boards.
• SNF or long-term care experience preferred.
• Proficient with occupational therapy practice and documentation.

Responsibilities

• Evaluate patients and assess impact on daily occupations.
• Develop individualized OT plans to maximize ADLs/IADLs.
• Modify home environments and recommend adaptive equipment.
• Deliver treatments per care plan and monitor progress.
• Collaborate with interdisciplinary team to ensure quality care.
• Complete documentation accurately and timely per policies.

Join Our Team as a (PRN) Occupational Therapist (OT)

We are seeking a compassionate and flexible Occupational Therapist (OT) to join our in-house team of outstanding professionals on a PRN basis. In this role, you will provide person-centered care focused on improving residents' independence, safety, and quality of life through meaningful daily activities and functional outcomes.

Your Impact as an Occupational Therapist (OT)

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life
• Assess and interpret patient evaluations and test results
• Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Assess patients' home and living environments and recommend modifications based on their needs and capabilities
• Recommend adaptive equipment and provide training to support independence and safety
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical education and support to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Active and valid Texas Occupational Therapy license
• In good standing with all regulatory agencies and licensing boards
• Skilled Nursing Facility (SNF) Or Long-term Care Experience Preferred
• Current knowledge of occupational therapy treatment practices
• Experience With Net Health Electronic Documentation System Preferred
• Ability to manage patients with varying personalities and needs
• Strong communication, collaboration, and clinical decision-making skills
