Job summary

FOX Rehabilitation is expanding in Columbus, IN, seeking licensed Physical Therapists to deliver one-on-one care to older adults in homes and senior living communities. The role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes within the Geriatric House Calls model.

Clinicians may incorporate telehealth and other virtual care to extend access and continuity of care, collaborating with PTs, OTs, SLPs, and Fitness Specialists to drive mobility and independence.

Qualifications

• Valid Physical Therapy license in the state(s) of practice
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences or senior living communities
• Deliver therapy sessions to improve mobility, independence and functional outcomes
• Document effectively in FOX's EMR system following regulatory standards
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists)
• Provide telehealth/virtual care when appropriate to extend access

• Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX's EMR system in accordance with regulatory standards
