Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist for in-home and senior living care in Bloomington, IN. You will deliver one-on-one therapy to older adults, promote mobility, and support independence across home and community settings.

The role emphasizes telehealth options, interdisciplinary collaboration, and scheduling flexibility while maintaining high-quality care and documentation in the EMR system.

Qualifications

• Current PT license in the applicable state(s).
• Degree from an accredited Physical Therapy program.
• Proficiency with EMR systems and basic computer tasks.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Deliver therapy sessions to improve mobility, independence, and quality of life.
• Document accurately in FOX's EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Offer telehealth or virtual care to extend access when appropriate.
