Job summary

Qualifications

• Basic computer skills required.
• Home Care roles require a valid state driver's license and minimum age, with MVR where applicable.

Responsibilities

• Interpret patients’ history and baseline functional level.
• Complete assessments appropriate to deficits.
• Establish and carry out OT plans of care.
• Evaluate and treat motor, cognitive, sensory integrative, and psychosocial function after provider referral.
• Reassess patients and modify plans of care as appropriate.
• Provide patient and caregiver instruction.
• Plan and coordinate discharge with the multidisciplinary team.
• Recommend durable medical equipment required for discharge.
• Communicate with physicians, nurses, case managers, social workers, and other team members.
• Document evaluations, plans of care, progress, discharge plans, and continuity of care.
• Maintain department productivity expectations.
• Supervise COTAs and rehabilitation technicians.
• Participate in quality improvement and team/committee meetings.
• Orient new staff members.
• Serve as clinical instructor for OT and OTA students.
• Educate other disciplines about OT purpose, goals, and procedures.
• Perform additional duties as requested by management.
