Job summary

Chatham Hills Subacute Care Center in New Jersey is seeking a licensed Occupational Therapist to join our rehabilitation team. You will assess referred patients, provide direct care, and develop treatment plans aligned with physician direction.

Responsibilities include setting achievable goals based on patient capacity, documenting progress, and educating patients and families on therapy methods, while supervising OT Assistants and collaborating with the multidisciplinary team.

Qualifications

• Must have graduated from an accredited OT program.
• Must hold valid state license/registration.
• Experience supervising OT assistants.
• Experience in long-term care/geriatrics preferred.
• Strong documentation skills.
• Collaborates with disciplines for best patient care.

Responsibilities

• Assess referred patients for rehabilitation needs and provide direct patient care.
• Establish and administer treatment programs with goals aligned to patient capacity.
• Document and record patient condition and progress.
• Educate patients and families on therapy methods.
• Collaborate with the Director of Rehabilitation and other disciplines.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: All positions are available.

Compensation: $43-47/hour
