Job summary

Chatham Hills Subacute Care Center is seeking a dedicated Occupational Therapist to join its rehabilitation team in a subacute care setting. You will assess referred patients, design and monitor treatment plans, and collaborate with physicians and nurses to optimize patient outcomes.

The role emphasizes patient-centered goals, detailed documentation, and education for patients and families, with leadership in supervising OT Assistants and ensuring regulatory compliance in a long-term care

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must hold a valid OT license and state registration.
• Experience supervising OT Assistants.
• Experience in long-term/geriatric care preferred.
• Strong documentation skills and clinical writing.

Responsibilities

• Assess referred patients to determine rehabilitation needs and potential.
• Establish and administer a treatment program with goals based on patient capacity.
• Document patient condition and progress, including weekly notes and discharge planning.
• Educate patients and families on therapy methods and expectations.
• Collaborate with all disciplines to provide patient-centered care.
• Adhere to confidentiality, HIPAA, and residents' rights.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: All positions are available.

Compensation: $43-47/hour
