Job summary

Family Tree Rehabilitation, located on The Campus of Anna Maria in Ohio, is seeking a PRN Occupational Therapist Registered (OTR) to support our therapy team across SNF, outpatient, and home health settings.

You will evaluate residents, develop and implement OT plans, document care, and collaborate with a multidisciplinary team. Ohio OTR license in good standing and compassionate, resident-centered approach are required.

Qualifications

• Current Ohio OTR license in good standing.
• Graduate of an accredited OT program.
• Experience in skilled nursing or rehabilitation preferred.

Responsibilities

• Evaluate residents and develop individualized OT plans of care.
• Provide skilled OT treatment per physician orders.
• Supervise and collaborate with COTAs as applicable.
• Document evaluations, treatments, progress and discharge plans.
• Participate in interdisciplinary care planning and team communication.
