Homeside Rehab is seeking compassionate Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) to deliver home health therapy across Fort Lauderdale, Coral Gables and Port St. Lucie, Florida.

Qualifications

• Active Florida Occupational Therapy License (OT) or Certified Occupational Therapy Assistant (COTA) license required.
• Minimum one year of experience in home health, outpatient, SNF, or related clinical setting preferred.
• Strong clinical reasoning and communication skills; able to work independently in patients' homes.

Responsibilities

• Perform comprehensive occupational therapy evaluations.
• Develop individualized treatment plans based on patient needs and goals.
• Provide evidence-based OT interventions to improve independence and quality of life.
• Educate patients and caregivers on treatment plans and home exercise programs.
• Maintain accurate and timely electronic documentation.
• Collaborate with physicians, caregivers, and team members for coordinated care.
• Monitor progress and modify treatment plans as appropriate.
