Job summary

Health Wealth Safe, Inc. is seeking an Occupational Therapist to evaluate and treat patients of all ages in both inpatient and outpatient settings.

You will design care plans, educate patients and caregivers, document progress, and collaborate with the care team on discharge planning. Illinois-licensed OT with BLS and inpatient/outpatient experience preferred; sign-on and relocation bonuses may be available.

Qualifications

• OT degree and state licensure in Illinois required.
• Basic Life Support (BLS) certification required.
• Experience in inpatient and outpatient care.
• Ongoing education related to stroke patient care.
