Concept Rehab is seeking a PRN Occupational Therapist to join our therapy team in Charlevoix, MI. You’ll evaluate patients, develop treatment plans, and help with activities of daily living, balancing independence with clinical excellence.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Michigan Occupational Therapist license.
• Strong clinical, communication, and organizational skills.
• Ability to work independently while collaborating with a team.
• Skilled nursing or post-acute rehab experience preferred but not required.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled OT services based on patient needs and goals.
• Help patients improve independence with activities of daily living.
• Address strength, coordination, cognition, positioning, mobility, and functional skills.
• Monitor progress and modify treatment plans as appropriate.
• Complete timely and accurate clinical documentation.
• Collaborate with nursing, therapy, physicians, patients, families, and facility staff.
• Participate in care planning and discharge planning as needed.

What You’ll Do

As an Occupational Therapist, you will:
