Job summary

Greenhills Health and Rehabilitation Center is seeking an Occupational Therapist to join our care team in Nashville, TN. You will assess patients, develop rehab plans, and provide direct OT services tailored to each individual's capacity and tolerance under physician direction.

You will document progress, educate patients and families, and work closely with other disciplines to deliver high-quality, compassionate care. Supervisory duties for OT Assistants may be required.

Qualifications

• Licensed OT with approved degree in Occupational Therapy.
• Current state license and registration to practice.
• Experience with long-term care and/or geriatrics preferred.
• Strong documentation writing and record-keeping skills.
• Ability to collaborate with multidisciplinary teams to deliver patient-centered care.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct OT care.
• Develop and administer treatment programs with clear goals.
• Document patient progress and discharge planning.
• Educate patients and families on therapy methods and goals.
• Supervise and guide Occupational Therapy Assistants as needed.
• Ensure confidentiality and compliance with HIPAA and residents' rights.

Occupational Therapist

Greenhills Health and Rehabilitation Center is looking for a talented and hard-working OccupationalTherapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
