Job summary

Greenhills Health and Rehabilitation Center is seeking a skilled Occupational Therapist to assess patients, design and manage rehabilitation programs, and work with a multidisciplinary team. You will document progress, educate families, and ensure care plans align with physician guidance.

Candidates should hold an accredited OT degree, a valid state license, and have strong documentation and patient-care skills. Experience with geriatric populations and supervision of aides is preferred.

Qualifications

• Graduated from an accredited Occupational Therapist program; eligible to practice.
• Valid state license/registration required to practice.
• Experience supervising OT Assistants preferred.
• Experience in long-term/geriatric care is beneficial.
• Strong documentation skills and record-keeping are essential.

Responsibilities

• Assess referred patients to determine rehabilitation needs and goals.
• Develop and administer treatment plans with physician oversight.
• Set realistic goals based on patient capacity and tolerance.
• Educate patients and families on therapy methods and progress.
• Maintain timely documentation: evaluations, progress notes, discharge summaries.
• Collaborate with all disciplines to ensure patient-centered care.
• Ensure compliance with confidentiality and residents' rights.

Occupational Therapist

Greenhills Health and Rehabilitation Center is looking for a talented and hard-working OccupationalTherapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
