Job summary

Greenhills Health and Rehabilitation Center is seeking a qualified Occupational Therapist to join our rehabilitation team on site. You will assess referred patients, develop individualized treatment plans, document progress, and educate patients and families under physician direction.

The role emphasizes collaboration with disciplines, adherence to HIPAA, and handling long‑term/geriatric care with a patient‑centered approach.

Qualifications

• Must have graduated from an accredited Occupational Therapist program.
• Must hold a valid OT license and registered status for the state of practice.
• Oversee and supervise Occupational Therapy Assistants.
• Experience in long-term/geriatric care preferred.
• Strong documentation skills and timely reporting.
• Adequate experience with evaluations, progress notes, re-certification and discharge docs.
• Collaborate with all disciplines to provide patient‑centered care.
• Demonstrate professionalism and punctuality at work.
• Comply with confidentiality and HIPAA; follow Residents' Rights.

Responsibilities

• Assess referred patients to determine rehabilitation needs and potential for improvement.
• Establish and administer a treatment program with goals based on patient capacity.
• Document patient condition and progress; educate patients and families.
• Work under physician direction to set realistic goals and adjust plans.
• Ensure all evaluations, progress notes, re-certifications and discharges are completed.

Greenhills Health and Rehabilitation Center is looking for a talented and hard-working OccupationalTherapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
