Job summary

Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join our rehabilitation team. You will assess referred patients, design and implement treatment plans, and educate patients and families while coordinating with physicians and the care team.

The role includes supervising Occupational Therapy Assistants, maintaining thorough documentation, and ensuring adherence to HIPAA and clinical standards in a long-term care setting.

Qualifications

• Graduated from an accredited Occupational Therapist program.
• Must hold a valid state OT license and registration.
• Experience supervising Occupational Therapy Assistants (OTAs).
• Long-term care and/or geriatric care experience preferred.
• Strong documentation and record-keeping skills.
• Demonstrated ability to collaborate with other disciplines and maintain patient confidentiality (HIPAA).

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop and administer treatment plans with specific goals based on patient capacity and tolerance.
• Document patient status, progress, and discharge planning; educate patients and families.
• Set realistic goals and monitor progress toward rehabilitation objectives.
• Supervise OTAs and ensure compliance with professional standards and confidentiality.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
