Job summary

Chatham Hills Subacute Care Center seeks an Occupational Therapist to join our rehabilitation team. The OT will assess patients requiring therapy, develop and supervise treatment plans, and educate patients and families while coordinating with the care team.

The role includes supervising OT Assistants, documenting progress, and maintaining compliance with confidentiality and regulatory standards within a long-term care setting.

Qualifications

• Must have graduated from an accredited Occupational Therapist program.
• Must hold a valid professional license and registration for the state of practice.
• Oversees and supervises Occupational Therapy Assistants.

Responsibilities

• Assess referred patients to determine rehabilitation needs and capabilities.
• Establish and administer a treatment program with specific goals based on patient capacity.
• Set realistic and achievable goals; document and track progress and outcomes.
• Educate patients and families in appropriate therapy methods; maintain clear records.
• Collaborate with other disciplines to provide patient-centered care.
• Ensure compliance with confidentiality, HIPAA, and Residents Rights.
• Punctual and professional conduct; adhere to scheduled hours.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
