Job summary

The Chatham Hills Subacute Care Center is seeking a dedicated Occupational Therapist to join our growing team. The role focuses on assessing referred patients requiring rehabilitation and delivering direct care.

You will establish and administer treatment programs with goals tailored to each patient, document progress, educate families, and supervise OT Assistants under physician direction. Collaboration with all disciplines and adherence to confidentiality are essential.

Qualifications

• Must have graduated from an accredited Occupational Therapist program.
• Must hold a valid state license and registration.
• Experience supervising Occupational Therapy Assistants.
• Experience with long-term/geriatric care preferred.
• Strong documentation/writing skills.
• Collaborates with all disciplines to provide patient-centered care.

Responsibilities

• Assess referred patients and determine rehabilitation needs.
• Develop and administer treatment programs with patient-specific goals.
• Document patient condition, progress, and discharge planning.
• Educate patients and families on therapy methods.
• Coordinate with physicians and other disciplines.
• Oversee and supervise Occupational Therapy Assistants.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
