Job summary

Chatham Hills Subacute Care Center in New Jersey is seeking a dedicated Occupational Therapist to join our rehabilitation team and improve patient outcomes. You will assess referred patients, develop treatment plans, supervise assistants, and collaborate with interdisciplinary teams while maintaining documentation and patient education.

The ideal candidate will have an accredited OT degree, current licensure in the state, and experience in geriatric or long-term care settings.

Qualifications

• Graduate of an accredited Occupational Therapist program.
• Licensed and registered to practice in the state.
• Experience supervising Occupational Therapy Assistants is preferred.
• Experience in long-term/geriatric care is a plus.
• Strong documentation and reporting skills.
• Knowledge of HIPAA and patient confidentiality.

Responsibilities

• Assess referred patients requiring rehabilitation to determine functional abilities and treatment needs.
• Develop and administer individualized treatment plans under physician direction.
• Set realistic goals based on patient capacity and tolerance; document progress.
• Educate patients and families on therapy approaches and home programs.
• Supervise and guide Occupational Therapy Assistants as needed.
• Collaborate with other disciplines to provide patient-centered care and continuity of care.
• Ensure timely, accurate documentation of evaluations, progress notes, and discharge planning.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
