Independence Rehab is seeking a Part-Time Occupational Therapist to join our SNF team in Utah. The role focuses on improving functional independence and safe discharge planning for geriatric patients.

Qualifications

• Geriatric care experience in SNF settings.
• Ability to provide adaptive ADL retraining.
• Proficient with all types of transfers (bed, chair, gait, etc.).

Responsibilities

• Provide occupational therapy services to geriatric patients in a skilled nursing facility.
• Address functional independence goals and safety to facilitate discharge planning.
• Collaborate with care team to support rehabilitation outcomes.
