Evolve Therapy Services, LLC is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Pahokee, FL. The OT will plan, direct, and provide therapy services, ensuring high-quality resident care under federal and state guidelines.

The role supports regular resident screenings, treatment planning, and collaboration with a multidisciplinary team. Licensure in the practicing state is required, with full-time, part-time, or PRN options available.

Qualifications

• Must hold an active Occupational Therapy license in the practicing state(s).
• Degree in Occupational Therapy is required.
• Ongoing continuing education and professional development to maintain licensure.

Responsabilités

• Screen residents to determine best intervention for each patient.
• Evaluate residents to inform treatment plans and goals.
• Communicate evaluation findings and plans to residents, families, and the team.
• Establish resident therapy schedules with team members.
• Deliver therapy per plan and ensure proper documentation and billing.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long‑term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
