Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Pahokee, FL. The role offers Full Time, Part Time, or PRN options, with responsibilities to plan, direct, and supervise OT services in the facility, ensuring high-quality resident care.

You will screen residents, develop treatment plans, communicate findings to the care team and families, and oversee therapy schedules and documentation, including COTA supervision and reimbursement practices,

Qualifications

• Active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Develop and adjust treatment plans with short- and long-term goals.
• Communicate evaluation findings to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules with the team.
• Document therapy services and ensure proper billing and reimbursement.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
