Job summary

Liberty Health Benson, North Carolina, is seeking an Occupational Therapist (OT) to join Liberty Commons of Johnston County on-site in Benson. You will evaluate patients, create individualized care plans, and lead therapy programs with a multidisciplinary team, including supervision of COTA’s and OT aides.

Requirements include graduation from an accredited OT program and NC Board eligibility, with SNF/long-term care experience preferred.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Must meet the NC Board criteria (new grads must be NBCOT-registered).
• Prior SNF/long-term care experience preferred.
• Strong clinical evaluation, communication, time management, and documentation skills.

Responsibilities

• Respond to service requests and obtain physician orders before evaluations.
• Evaluate patients and write individualized plans of care with measurable goals.
• Assess treatment effectiveness and modify plans as needed.
• Establish restorative/maintenance programs tailored to patients.
• Plan, administer, and supervise therapy programs and supervise COTA's and OT aides.
• Communicate with physicians, rehab team, and staff about total programs.
• Maintain accurate daily treatment records and weekly progress notes.
• Follow Liberty Long Term Care policies and procedures.
• Maintain workload with productivity rate of 75% or higher.
• Prepare and organize the work environment and therapy equipment.
• Conduct facility in-services as requested.
• Travel to other locations to provide therapy services.

We are currently seeking an experienced:

OCCUPATIONAL THERAPIST

Work with a multidisciplinary team to ensure a wonderful rehab experience for patients and their families by being responsive and individualizing treatment plans. Thrive in a supportive rehab environment with progressive treatment modalities to address a variety of diseases and impairments. In-house therapy with local management support.

JOB SUMMARY:

• Respond to requests for service and obtain physician orders prior to patient evaluations.
• Evaluate patients and establish individualized, written plans of care featuring objective, measurable, and functional goals.
• Assess treatment effectiveness regularly and modify plans of care as clinically indicated.
• Establish restorative and/or maintenance programs tailored to individual patient needs.
• Complete special requested patient screens as needed.
• Plan, administer, and supervise therapy programs, including the supervision and training of Certified Occupational Therapy Assistants (COTA’s) and OT aides.
• Communicate effectively with referring physicians, interdisciplinary rehab team members, and facility staff regarding total treatment programs.
• Maintain accurate, timely, and complete daily treatment records and weekly progress notes in accordance with documentation guidelines.
• Follow all established Policies and Procedures of Liberty Long Term Care.
• Submit all required corporate employee paperwork in a timely manner.Maintain a patient workload compatible with the work setting, upholding a productivity rate of at least 75% (or higher as deemed necessary).
• Prepare, organize, and maintain the work environment, therapy equipment, and supplies.
• Conduct facility-requested in-services as requested.
• Travel to other locations to provide therapy services as needed.

JOB REQUIREMENTS:

• Graduate of an accredited Occupational Therapy program.
• Must meet the criteria of the North Carolina Board of Occupational Therapy. (New graduates must be registered with the National Board for Certification in Occupational Therapy.)
• Prior experience in a Skilled Nursing Facility (SNF) or long-term care setting preferred.
• Strong clinical evaluation skills, effective communication, sound time management, and proficiency with healthcare documentation platforms.
