Evolve Therapy Services in Randallstown, MD is seeking a dedicated Occupational Therapist for an in-house Skilled Nursing Facility. This part‑time or PRN role focuses on delivering high‑quality OT services within a team‑based care setting, coordinating with residents, families, and staff to optimize functional outcomes.

You will assess residents, develop treatment plans, supervise COTAs, document care and ensure compliance with standards.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine appropriate interventions.
• Evaluate residents to develop treatment plans with goals and methods.
• Communicate evaluation findings and plans to residents, families, and the care team.
• Establish resident therapy schedules with other team members.
• Deliver therapy per plan and ensure proper documentation and billing.
• Perform other duties to effectively treat patients as required.
• Oversee COTA and ensure documentation and treatments meet standards.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Part Time or PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
