Job summary

Evolve Therapy Services, LLC seeks an Occupational Therapist for an in-house Skilled Nursing Facility in Holly Hill, FL. This PRN position focuses on delivering OT services within a collaborative interdisciplinary team and ensuring high quality resident care.

Responsibilities include conducting resident screenings and assessments, developing treatment plans with goals, documenting progress for reimbursement, and supervising COTA staff while maintaining licensure and continuing education

• Evaluate residents to inform treatment plans and goals
• Interpret and communicate findings to residents, families, and team

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
