Job summary

Evolve Therapy Services is seeking an Occupational Therapist to provide in-house therapy services at our partner Skilled Nursing Facility in Southport, CT. The role focuses on developing and directing OT plans of care, coordinating with an interdisciplinary team, and ensuring documentation and billing accuracy.

The ideal candidate holds an OT degree and an active state license, commits to ongoing professional development, and is willing to supervise COTA as needed.

Qualifications

• Must have an active Occupational Therapy license in practicing state(s).
• OT degree from accredited program.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Screen residents to determine best intervention.
• Evaluate residents and develop treatment plans with goals.
• Communicate findings to residents, families, and the team.
• Establish therapy schedules with the team.
• Perform therapy per plan and ensure documentation and billing.
• Oversee COTA and ensure documentation and reimbursement practices.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
