Job summary

Evolve Therapy Services, LLC is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in New Haven, CT. The role is full time or PRN, focusing on planning, organizing, and directing OT services to maintain high resident care.

The OT will screen, evaluate, set goals, and develop treatment plans, while coordinating with families and the interdisciplinary team and supervising COTA as needed.

Qualifications

• OT degree required and active license in practicing states.
• Ongoing continuing education to maintain licensure.
• Ability to oversee therapy Team and ensure quality care.

Responsibilities

• Screen residents to determine the best OT interventions.
• Evaluate residents and develop treatment plans with goals.
• Communicate findings and plans to residents and families.
• Schedule individual resident therapy sessions with the team.
• Deliver therapy per plan and document/bill services.
• Supervise COTA and ensure compliant care and documentation.
• Perform other duties as needed to treat patients.

Job Position

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type

Full Time or PRN

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
