Evolve Therapy Services, LLC seeks an Occupational Therapist for an in-house Skilled Nursing Facility in Durham, NC. The role plans, directs, and delivers occupational therapy services, ensuring high-quality resident care in accordance with standards and regulations.

The position involves screening, evaluating, planning, and coordinating therapy with the interdisciplinary team, plus supervision of COTA and accurate documentation and billing.

Qualificações

• OT degree required.
• Active OT license in practicing state(s) required.
• Ongoing continuing education to maintain licensure and certification.

Responsabilidades

• Conduct regular screenings to determine best intervention for each patient.
• Evaluate residents to develop treatment plans and set short- and long-term goals.
• Communicate evaluation findings and plans to residents, families, and the interdisciplinary team.
• Create individual therapy schedules in collaboration with the care team.
• Deliver therapy per plan and ensure proper documentation and billing.
• Oversee COTA and ensure compliant documentation and treatments.
• Perform additional duties to effectively treat patients.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
