Evolve Therapy Services is seeking a licensed Occupational Therapist to lead therapy services in an Olivia, MN in-house Skilled Nursing Facility. You will develop and direct OT programs, screen residents, and coordinate with care teams.

This opportunity offers either full-time, part-time, or PRN scheduling, plus a $5,000 sign-on bonus and tuition reimbursement. A valid Minnesota OT license and ongoing CEU commitment are required to ensure high-quality resident care.

Qualifications

• Must hold an active Occupational Therapy license in the practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsabilités

• Conduct regular screening of residents to determine the best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and the interdisciplinary team.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions
